Confidential Client Information: If you are scheduling a Pastoral Biblical Counseling appointment, print out and complete the Confidential Client Information form (CCI). Then bring your completed form to your appointment. Click the PDF icon on the right to view and print the CCI form. If you are unable to print the form, please arrive 20 minutes early for your appointment to complete the paperwork.

Consent to Christian Biblical Counseling or Discipleship: Review the Consent form before your first appointment for Counseling or Discipleship. You will discuss and sign the form at the office. Click on the Review Consent icon on the right to view the Consent form. You may also print it is you like.
